The terms NUCAP or ‘us’ or ‘we’ or ‘our’ refers to the owner of the application. The term ‘you’ refers to the user or viewer of our application.

Please read the Terms of Use carefully before you start using your account. By registering for use of this App, you acknowledge that you are at least 18 years of age and agree to be bound by these Terms of Use.

Please check these Terms of Use and Privacy policy mentioned in this Agreement.

General Provisions:

The use of this application is subject to the following terms of use and privacy policy:

• The content of the pages of these applications is for your general information and use only.
• These are not lifesaving applications or devices. Please call your health care professional or emergency if you do not feel well.
• NUCAP is a dashboard to present study progress for research progress as the subject reaches various study milestones. These insights hope to improve participant engagement, improve subject retention, and create a feedback loop for the study subject. These research data are housed on an electronic data capture system called REDCap and is extracted for NUCAP.
• You will receive a user account for our personal use that requires a username and password. You are responsible for the accuracy, quality, integrity, legality, reliability, and appropriateness of all data you provide, and the recommendations made by the applications will be impacted if the information provided is not correct.
• You may request to terminate this agreement anytime with a written notice to Research Team. Northwestern University will process the termination of the User Account within 5 days of the request.
• Northwestern University reserves the right to refuse activation of a user for any reason.
• Northwestern University may deactivate, terminate, disable, suspend, or revoke access to any or all features of the App if there is a breach of any term mentioned in this agreement. Northwestern University will deactivate if there is any abuse of the application or if the user is adversely using any feature.
• The App is enabled by technology, software and certain content delivered electronically. You agree that you will not reverse-engineer, decompile, disassemble, translate, or otherwise attempt to obtain access to the source code of, any aspect of the App. You will not copy, reproduce, alter, create derivative works from, or otherwise modify the App. You will not lease, loan, sublicense, distribute, or otherwise provide others access to or with any aspect of the NUCAP App except to an authorized user. Other than as expressly set forth in this Agreement, no license or other rights of the App are granted to you, and all such licenses and rights are hereby expressly reserved.
• We have our App designed with security first. We seek to safeguard the security of your Personal Information data and have implemented reasonable security measures consistent with applicable laws, and accepted practices in the healthcare industry to protect the confidentiality of your Personal Information. All sensitive data at rest and data in transit are encrypted with industry-standard formats. Despite our efforts, however, we cannot guarantee the absolute security of your Personal Information, nor can we guarantee that information that you provide will not be intercepted while being transmitted to us over the Internet. We urge you to also take every precaution to protect your Personal Information when you are on the Internet or using the App. In order to protect your privacy, never share your sign-in name or password and always log out of the App when you are finished using the App.
• We recommend and highly discourage you to send e-mail communications containing personal health information to us. We cannot secure any Personal Information sent by e-mail because such information can be accessed by other internet users.
• When any of the terms are changed, modified, and/or amended, the revised terms will be updated on our app and the effective date will be updated too. By accepting this, you accept any such modifications to the terms / Policy by continued use of our app after such modifications are made. As a user, you have the right to refuse and reject such amendment for 30 days prior to its effectuation and terminate the agreement.
